Please read carefully and fill out all of the forms below. Forms must be filled out completely before the patient can be seen At this time we are only accepting patients via telemedicine in the state of Virginia and Florida. Payment is due in full upon booking appointment. Full payment will be refunded if cancellation is made 24 hours in advance. No insurance is accepted, however, we may provide with a receipt upon request that you may submit to your insurance company for reimbursement, however, we do not accept Medicare. In addition, you are not eligible to receive services from us if you are a Medicare recipient, due to Medicare rules. The patient must have access to a computer with web camera and privacy. The patient must also have a blood pressure machine in working order. For the best connection, it is recommended to be connected to a LAN. The session may not be recorded by either party for privacy. 30 minute follow up (must have initial appointment first) - $155.00
15 minute follow up (as recommended by provider) - $75.00
