What should I do if I have suicidal thoughts or feeling suicidal?
You must call 911 or go to the nearest emergency room immediately if you have suicidal thoughts or feeling suicidal. In addition, you can call the National Suicide Prevention Lifeline: 800-273-8255 or contact them via live chat: National Suicide Prevention Lifeline. Our services are not for emergencies. For all other questions in-between your appointments, contact your primary care provider.
Do I have to sign up for the initial evaluation if I want to become a new patient?
Yes. Everyone wanting to become a new patient must complete the initial live video (or in-person) evaluation. You also must complete and sign all your forms prior to your first appointment.
Can I be a resident anywhere in the US?
No, you must be a resident in a state where the Psychiatrist or PMHNP (Psychiatric Mental Health Nurse Practitioner) is licensed to practice. You must provide current proof of residence via a valid government-issued ID. Please see each specific provider to see where they are licensed.
Are there any age limitations on becoming a patient?
No, but the parent or guardian of a patient under the age of 18 is required to be present during the whole appointment or as directed by the healthcare professional.
Do you accept my insurance?
No. All payments must be made online via Apple Pay, Pay Pal, Venmo or Zelle prior to your appointment.
Do you accept Medicare?
No. We do not accept Medicare. In addition, you are not eligible to receive services if you are a Medicare recipient due to Medicare rules.
What is your cancellation policy?
All new patient appointments must be cancelled or rescheduled at least 24 hours prior, in order for a refund to be processed. All current client appointments must be cancelled or rescheduled 24 hours/1 business day (e.g. before 10am Friday for a 10am Monday appointment) in order for a refund or time credit to be processed.
Are there any medications you do not prescribe?
YES. We do not prescribe Oxycotin, Oxycodone, Hydrocone, etc and Subutex. We do NOT combine Suboxne, (buprenephrine/naloxone) with any benzodiazepines including but not limited to: Xanax (alprazolam), Ativan (lorazepam), Klonopin (clonazepam), Valium (diazepam) or Ambien. You must be seen before a determination can be made if these medications are right for you even though you may be prescribed these medications by another provider. We require a physical examination before you are seen, some may need a medical clearance from a healthcare professional before we can provide care and prescribe medications. We require recent blood work and urine drug screens. Patients must also have a home blood pressure monitoring machine or three recent blood pressure readings from another means. The patient must agree to random urine drug screen testing. We also do not guarantee you will be prescribed certain medications.
Prior Authorization for Medication
Many insurance companies require prior authorization on certain medications. Please have your pharmacy fax the notification to our office and we will respond to the request from your insurance company. Keep in mind that prior authorization requests can take from 2-5 business days to process after we send the information to your insurance carrier. We are not responsible for your insurance company's decision of whether or not they will cover your medication.
If you are seeking help with access to medications through a special program; ( i.e. patient assistance), we will provide the prescription to you. Our policy is that you keep up with the 800 numbers for the programs so that you may follow up with your request. We cannot guarantee how long programs take to consider your request.
Letters & Forms
If you are requesting a letter to be written on your behalf or forms to be completed, please keep in mind that your request will take 3+ business days to complete. We may charge for phone calls and paperwork requested that require more than 10 minutes of our time, billed in 10-minute increments of our hourly fee. Our healthcare professional must approve and sign all letters before they can be mailed/faxed. The minimum turn around time for letters is one week, so please request them 1-2 weeks in advance.
Disability Claims
We do not fill out disability claims, nor do we evaluate for them.
